The Al-Wustho Mosque was originally built during the reign of KGPAA Mangkunegara I, and later relocated to the west side of the Mangkunegaran Palace during the era of KGPAA Mangkunegara IV. Formerly known as Masjid Nagari, the mosque was renamed Al-Wustho Mosque in 1949 by Raden Tumenggung KH. Imam Rosidi, the palace's head religious leader (penghulu).

Initially, the mosque was managed by Mangkunegaran royal courtiers (abdi dalem). However, after Indonesia’s independence, its management was transferred under the supervision of the Ministry of Religious Affairs of the Republic of Indonesia.

Architecturally, the mosque showcases a fusion of Javanese and European styles. One of its most distinctive features is the ornate gateway (gapura) adorned with Arabic calligraphy, including an inscription that reads:

“Whoever builds a mosque for the sake of Allah, Allah will build for him a house in Paradise.”
